Medical Dental Vision – Employer sponsored health benefits will end at the end of the month in which employment ends. (Example: 5/15 term date = covered until 5/31).
Life, LTD, Critical Illness, Accident and Met Life Legal – End the day of termination
COBRA
Under the COBRA federal law, an employee may choose to continue the medical, dental, and/or vision coverage upon termination of employment. The employee will be responsible for paying the total monthly health, dental or vision premium plus a 2% administrative fee. To continue the health care FSA you must continue to make your regular monthly contributions after-tax to the COBRA administrator. FSA Funds will not rollover to the following year.
Electing COBRA Coverage – A COBRA packet will be mailed within 15 business days of the coverage end date. The employee will have 60 days to make a COBRA election. If COBRA is elected, the coverage will be retroactive back to the date the employee lost coverage. For questions regarding COBRA please contact HR Pro at 248-543-2644 option #3.
Life Insurance Portability/Conversion
With Sun Life Financial Group Life employees can take their coverage with them by porting or converting. Portability and conversion help employees keep coverage when they are no longer eligible for the employer-sponsored group plan. To learn more about how portability and conversion, please see links. 401k Fidelity
Contributions to your 401k retirement plan ceases upon termination. As a plan participant, you may have the right to receive benefits when you reach your normal retirement age under the plan or terminate employment, provided your plan account has monies in it available for your withdrawal.
