Leave of Absence
INSTRUCTIONS
This document initiates the leave process for all members of the Inotiv team for the following leave types:
- FMLA/ADA
- Short Term Disability
- Paid Parental Leave
- Military Leave
The employee’s HR Representative must complete Section 1 and send to the benefits mailbox usbenefits@inotiv.com as soon as possible. Allow at least 7 days before the employee’s last day of work.
Upon receipt of this form, the HR Benefits Team will complete an in depth leave review with the employee. This will cover important information regarding the employee’s responsibilities while on leave, their benefits, pay and return to work process.
If you or the employee have any questions regarding leave claims or requirements, you are encouraged to contact the HR Benefits Team via email or Sun Life at 877-786-3652. For Paid Parental Leave, contact HR only.
